· Introduce yourself to Doris and gain consent to carry out her care.
· Explain what you are going to do to Doris, give as much information as you can, and make sure Doris understands what is going to happen.
· Gain permission to share relevant information with other significant professionals
· Discuss Doris' view, objectives and desired outcome (Person Centred)
· Wash hands using the washing hand techniques to prevent introducing health care associated infections (HCAI) and use alcohol gel when possible.
· Wear protective clothing.
· Help Doris to sit in a comfortable position in order to examine her wound making sure she was fine.
· Assess Doris's wound, making sure it's not oozing
· Look at the wound, noticing the location, colour the size of the wound.
· Measure the wound and take pictures for further reference.
· Check whether the 90% slough skin has changed whether is there any increased exudates and the wound is still smelly.
· Check whether the wound is still 10% granulating or its over granulating.
· Check wound for signs of infection, swelling or inflammation
· Check whether the wound is infected, if so, take a wound swab for further checks.
· Clean the wound with normal saline solution.
· Look for an appropriate dressing for Doris's wound, considering the following
- Appropriate size of dressing
- Availability
- Easy of application and removal
- Skin friendly
· Ask Doris's GP to review Dori's medication and prescribe antibiotics if there are any signs of infection.
