Assignment title: Information
Ms Edna Forks is an 82 year old female who is being discharged after a five day hospital stay for hypertension. Her test results are all 'NAD' according to her discharge summary. She does need to make an appointment for an outpatient CT Abdomen and also needs to have a blood test taken to review her lipid profile. Ms Forks needs to also see her General Practitioner (GP) for ongoing management and treatment for her hypertension. Ms Forks is partially deaf, requiring the use of bilateral hearing aids. She wears glasses for reading and uses a walking stick for mobilisation. Ms Forks lives alone and has no children or family near her. When you arrive at the bedside to speak with Ms Forks, you notice she is muttering under her breath. When you ask her what she is saying she says, quite loudly – "This place is ridiculous, I have been asked to get out already by someone as they want my bed for someone else, but I have not had anyone to help me pack up my things and I don't know what I am supposed to be doing. I have new medicines to take and these ones here have someone else's name on them – I might be deaf but I am not blind!" Students will be assessed on the following skills in their reflection. Respond to the following questions using your interactions with Ms Forks as your basis for your answers. Using the communication skills you utilised in the reflective practice, how have you evaluated the situation? Consider the following points in your response – Analysis of your discussion with Ms Forks. Consultation methods – who did you need to refer to in order to meet Ms Forks' needs? If you did not consult anyone, is there a specific health care professional that you would in hindsight consult? Negotiation – was there any negotiation required in your discussion with Ms Forks? Time management of the scenario. Did you find that the time went quickly? Did you cover enough information? Did you respond to cues from Ms Forks appropriately?
