1. Mrs Holt, 85 years old, has been admitted to acute care following a fall resulting in a fractured femur. She is a widow and lives alone with her three cats for company. a) What aspects would you consider in preparation for her discharge? In your answer consider what the discharge planning process includes and who is involved. (100 words) b) Research and discuss a minimum of three (3) services that are available to assist the older person with maintaining social contacts within your local area. (100 words) Im located in Sydney Answer help : a) In your response briefly explain when discharge planning should begin and what discharge planning involves (see Calleja, 2013, p. 258 – details on the Reference list). Then consider Mrs Holt's circumstances and the aspects of her care she may need assistance with at home. Include the members of the health care team, in the facility and in the home, who may be involved in her case (see Commonwealth of Australia, 2011 or Rebeiro, 2013, p. 270). b) Research provisions made for older people in your area. Remember this is about maintaining social contacts. What do the services offer and how can the older person access them? (E.g. provide a contact phone number/email to demonstrate your research) 2. Mrs Munyarryun, 67 years, has been retired from her work for two years. She rings for advice about urinary incontinence, a problem she has experienced over the last 6 months. a) What might have triggered urinary incontinence? (100 words) b) What would you include in your advice? (100 words) Answer help : Provide a general explanation of what urinary incontinence is and research the common triggers of the condition in the older person. Then research the recommended treatments and explain the advice and education you would give the client about the condition (see Watt, 2013, p. 1157 and Dowling-Castronova, 2013). 3. Mr Konarski, 73 years old, lives in a high-level care facility. He has dementia of the Alzheimer's type, osteoarthritis in both knees and ankles, and hypertension, which is controlled by medication. He requires supervision and help with all personal care. Today, however, he appears unusually agitated and restless. Consider Mr Konarski's medical conditions and explain how they might have contributed to this change. (200 words) Answer help: Think about all the conditions the client has. You will need to do some independent research to find out if there's anything associated with these conditions which may affect mood and behaviours. For example, you could research changed behaviours in people with dementia and investigate what happens as the disease progresses. Also, you could examine the pain associated with osteoarthritis. What happens if the pain is not managed properly?
