You are the Registered Nurse (RN) on a morning shift in the Emergency Department (ED) of a regional hospital. You commenced your shift at 0700 hours on Sunday 21 February 2016, and you have been allocated to care Mr Chris Callaghan who has history of pancreatitis and diabetes. Consider the case study below answer the following questions. This plan of nursing care should have the nursing care priority order for your shift based on the information provided to you. This plan of care should also include a primary and secondary assessment, allocation of a triage score and ethical / legal issues. Also include the nursing interventions to assist family members or significant others cope with Chris's illness and hospitalisation (1500 words) Case Study Date: Sunday, 21 February 2016, Time: 0730hours Chris Callaghan is a 36-year old male miner brought into hospital by the Queensland Ambulance Service (QAS) officers following a telephone call by Chris's friend (John). John found Chris lying semi-conscious on the front veranda in a pool of blood and vomit at 1330hours this afternoon. John further explained to the QAS officers that he was surprised to discover Chris on his front veranda as he had left the football party at around 2300 hours the previous night. John also stated that Chris had ingested a large quantity of alcohol and hamburgers with hot chips at his football party the previous night. Chris has blood on his face from an 8cm long laceration above his left eye and bruising to the left side of his face. Chris has complained of a headache, tasting blood in his mouth, severe epigastric pain that is radiation to his back and he is also feeling cold. Chris cannot remember how he sustained the laceration and the bruises to his head. The QAS officers advise you that Chris's epigastric pain seems to be somewhat relieved on leaning forward. The officers state that Chris has a past medical history of gastric ulcer, pancreatitis, smoking (current – up to a packet/day) and sulphur allergy. The QAS officers state that Chris's last observations were: respirations 26 breaths/minute; temperature 38.30C; BP 160/72mmHg; SpO2 96% at Room Air (RA); PERRL 3mm, but that his GCS is 14 (disorientated to time and place). The QAS officers attached the NURS13126 Critical Care Nursing – Term 1, 2016 following ECG rhythm strip to clinical report: The QAS officers estimate Chris's weight to be 85kg, and his height to be 183cm. An intravenous infusion of 0.9% normal saline (500mL; running at a rate of 250mL/hr) was commenced by the QAS officers prior to ED arrival through a peripheral cannula in the Chris's cubital fossa. QAS officers state that Chris's abdomen revealed hypoactive bowel sounds in all four quadrants on auscultation, abdominal distension and tenderness, and guarding throughout with light palpation. Following your initial introduction and assessment, Chris states he is feeling nauseous and has three episodes of vomiting since John found him. Your further assessment reveals Chris has evidence of anxiety and teary while talking to you. Chris asks you where he is, what the time is and what day it is. Chris verbalizes generalized abdominal pain and has a history of pancreatitis. First diagnosis of pancreatitis was made 5 months ago, and the last episode of pancreatitis was 2 months ago requiring hospitalization for 6 days. Chris also has a history of type-1-diabetes mellitus and is currently on short acting insulin (8 units before each meal). Chris states he must be discharged from the hospital by 1300 today so he can pick up his two girls, Kate aged 8 years and Sharon aged 10 years from school. He also mentions that is divorced and it is his week to have the children, and his ex-wife, Annette has threatened to return to court regarding custody of his two girls if he misses this visit with the girls. Chris has requested that he does not want Annette to learn of this NURS13126 Critical Care Nursing – Term 1, 2016 hospitalisation as he thinks she will use this as evidence against him in the custody case in court.
