Background: Greg Price, 31, died on May 19, 2012, three days after surgery to remove a cancerous testicle. After an extensive inquiry into the events leading to Greg's death, it has been found that he suffered from multiple communication gaps among his caregivers, including a three-month wait for a consultation to see a surgeon without any notifications, and the failure of a diagnostic imaging clinic to inform a referring physician about the results of a CT scan. The Health Quality Council's report, called Continuity of Patient Care Study, makes 13 detailed recommendations. You are the lead analyst of "VitalConnect" and are responsible for the requirement specification and technology evaluation. The selected application(s) will facilitate seamless communication among physicians at different care facilities, as well as communication between physicians and patients. You are asked to provide a document containing the following key components: Outline the gaps in the patient information exchange that are attributed to Greg's death. Your analysis should be two-fold: the missing of vital health informatics solutions (such as a patient portal) and the deficiency in the processes (such as the definition of "time-sensitive condition") (3/15 marks) Identify stakeholders in the "VitalConnect" program and specify their roles and responsibilities (2/15 marks) Describe the application systems that need to be procured, list high-level function requirements for each system, draw a diagram to illustrate the desired connection between these systems and key messages/data need to be transmitted and present the evaluation matrix for the "VitalConnect" program (10/15 marks)
