Dylan, you look very stated. How are you feeling now? From the record, the nurse can see that that he had applied for leave because he was not getting sleep. What is your feeling - depressed, discouraged or low? It is also reported that you heard some sound. What comes to your mind when you hear this sound? How do you react after hearing this sound, like irritable, edgy or angry? You had described this notice sometimes as scratchy or sometimes as muffled. What makes you feel like this? What is bothering you after hearing this notice? Do you have any other personal beliefs that have not shared before? After this incident, have you ever felt removed/detached/different or changed from others surrounding you? How these sounds are appearing to you-unreal or unnatural? You had shown concerns about food poisoning. Have you ever thought that some group or someone is intended to harm you in this way?
Answer to Question 2
Mental health assessment is a very crucial part in psychiatric practice (McCloskey & Perkins, 2012). In Dylan’s case, it is seen that he can hear some noise in the ceiling. Therefore, he cannot be able to sleep. This incident is bothering him continuously as he applied for sick leave. Therefore, a mental health assessment is very necessary to know his mental condition. His mental state is assessed under the domains of judgment, insight, cognition, perception, thought content, thought process, speech, affect and mood, behavior, appearance in a structured way (Armstrong et al., 2015). The history of the present mental illness is expressed through this assessment. Checkpoints on some mental problems like anorexia nervosa, disease, schizophrenia, depression, anxiety can be done by doing this assessment. This diagnosis report is also necessary for Dylan so that he can apply for insurance to bear his treatment fees. The main aim of this practice is to evaluate response to the treatment, plan effective care, and facilitate diagnosis and to establish a baseline. In Dylan’s case, it is also seen that he does not want to use medicine. Therefore, this treatment helps him to share his problems and gives him some mental relief.

Appearance and Behavior:
Appearance gives important clues to a person's normal physical health, motor condition, the presence of thought disorder, self-awareness, cognitive state, mood (Weber & Kelley 2013). Some unusual or prominent physical characteristics (Dylan's short hair, soiled cloth), approximate weight and height (Dylan's tall and slim appearance), posture, hygiene and grooming (not shaved for several days), level of eye contact (Dylan's was towards his feet) etc give the ideas of appearance in this examination. Dylan's attitude towards interviewer and interview will represent his behavior or attitude as he was not agreed to sit for this interview. His behavior can influence the content or validity of obtained information, and it also gives some challenges to the interviewer. Dylan's behavior towards the nurse can be described as threatening, hostile, guarded or evasive, defensive, deductive, frank, uncooperative or cooperative. Changes in this attitude in the interview process will also be recorded
