Case Study 2: Lucy Liu HISTORY: Lucy Liu is 19yrs old. She is at university and lives alone in student accommodation. She has no close relatives living in Australia. Lucy likes to socialise with her friends on a Friday night. She reports that she “sometimes drinks too much alcohol” and takes “party drugs”. She has a part-time job, 3 evenings a week, at a supermarket restocking shelves. Lucy presented to the emergency department (ED) with acute appendicitis. Her symptoms included severe nausea and vomiting, severe pain, tenderness and guarding in the right lower quadrant of her abdomen and a temperature of 38.9oC. She was transferred to the operating theatre and underwent an emergency laparoscopic appendicectomy. After 2 hours in the post-anaesthetic recovery room (PARU) and an uneventful recovery, Lucy was transferred to the ward, where you have been allocated to her care. PRESENTATION TO THE WARD: On return to the ward Lucy’s observations are as follows:  Pain 8/10  Respirations 26  BP 90/50  Pulse 130  Temperature remains 38.9oC. It is planned for Lucy to be discharged two days post-operatively. Questions to be addressed In relation to Lucy Liu:  Discuss the factors that may have contributed to the patient’s presenting episodic health alteration.  Critically discuss appropriate nursing management of the patient during this episodic health alteration.  Explain the role(s) of the key member(s) of the interdisciplinary health care team that you would involve in the care of your patient, and provide justification for their involvement. Please try and keep similarity below 10% by reading widely & paraphrasing the research/evidence.  Although we suggest you do your background reading in the current textbooks for basic information, the case study also requires you to find current literature/research/articles to support your discussion throughout the case study.  Do NOT use Better Health Channel, WedMed, dictionaries, encyclopaedias etc. These are consumer sources and are NOT suitable to support academic discussion. You may use government policies, clinical guidelines, UpToDate, systematic reviews, journal articles & original research.  You must follow the APA referencing format in your case study and in your reference list (Quick Guide in Case Study folder on LEO). You can find examples of the correct format at the end of your lectures and tutorials also.  You must follow the format for setting out your assignment re font style & size, margin width and spacing (Guide in the Case Study folder on LEO) Do not put references in your reference list that are either not in your case study (we do check) or are irrelevant e.g. from veterinary journals. Try to find evidence (research policies & human) from countries with health care systems similar to Australia.  The case study should have approximately 10 relevant sources. Textbooks, if cited, should be a range of medical-surgical, pathophysiology, anatomy & physiology & pharmacology for specific information. Some of these textbooks are available via the Library website in ClinicalKey. You need an introduction a conclusion, to write in sentences and link ideas into paragraphs. Do not write in point form as this does not show critical thinking. Do not use abbreviations such as e.g. i.e. etc. in yourPage 2 of 2 essay and you must explain terms not just copy them out of a book. The marker needs to know you understand the condition & care of the patient for safe practice. Specific Guidance for the Case Study  Please review the rubric before you commence your essay.  Briefly discuss your chosen patient’s presenting condition in relation to their diagnosis of acute cholecystitis or acute appendicitis, the reasons the patient may be susceptible i.e. risk factors.  Discuss the changes the patient is experiencing post-operatively why they might be experiencing these changes considering their presenting condition and their surgery.  Discuss how you would you manage these changes.  Identify which members of the MDT you could refer your patient to during their admission.
