Your manager has asked you to visit a residential setting for individuals for specialist dementia care provision for adults diagnosed with conditions like Alzheimer's, which has failed a recent inspection. You are asked to deliver a presentation to the staff, on partnerships and relationships in health and social care. Health care staff available in the residential care home include: Admiral Nurses, doctors, care home managers, health care assistant, psychologist, nutritionist, Lifestyle and beauty team and physiotherapist. The health care professionals work in various multi disciplinary teams with the aim of working in collaboration with each other, service users and families. The hospital’s management team has embraced the principles of collaborative working and has urged all team members to work effectively. The team is made up of community psychiatric nurses (CPN), occupational therapist, psychologist and social workers. Territorial disputes arise in the multidisciplinary approach about roles and responsibilities between the local community mental health team and the mental health trust staff. The clients and families have complained to CQC (Care Quality Commission) about the poor services rendered by the mental health Trust. Patients have complained that the staff members do not communicate effectively, lack of information sharing, poor support and generally poor quality of care. Q3 Using the above case study 1 and other factors learnt on this course, devise strategies to improve partnership working in health and social care. He has a case load where a client Mrs Mary is 68 years old, barely being maintained at home and desperately in need of permanent nursing placement. The client’s family are putting Mr Joe under constant pressure to get it sorted as soon as possible. Mr Joe has carried out assessment and completed all paperwork and the funding has been approved. The issue is there are no nursing beds in social services contracted homes within 40 miles radius. There is a private nursing home but their fees exceed the social services benchmark figure. Mr Joe has explained on several occasions to the family that the private bed can’t be used unless the family wants to top up the fees. The District nurses visit Mrs Mary regularly. They are also feeling the pressure and understand the situations perfectly well but when confronted by the increasingly depressed family members, they occasionally lapsed into saying things like “if only social services pulled their fingers out”. The District nurses have complained to the community health partnership unit. The nurses feel there is a miscommunication and lack of understanding to the severity of the health condition of Mrs Mary. The issue is the client and is yet to be resolved.
