The question asked you to Post details of your company's formal (i.e. written) Governance structure, or post details of your company's processes that exist to ensure efficiency, control and value as well as accountability and responsibility, if your company does not have a formal structure.
IT governance has two distinct components. There is a structural component that pertains to the organisation’s information technology activities, the way those activities support the goals of the business, and the people who help manage those activities. The process defines the decision-making rights associated with IT as well as the mechanisms and policies used to measure and control the way IT decisions are made and carried out within the organisation.
You must describe each section. This usually covers corporate structure(s), who are they? What are their roles and responsibilities in decision making process to meet business goals? Process covers the process of request, proposals and approvals in the decision making process. Communication covers the how the communication flows between all stakeholders on various projects in decision making process. You can check the company organisation chart as a starting point.
With the IT or Corporate governance is actually posted in your website under ‘About Us’ page then click on “Management” link. Here it talks about the company governance; where the management is comprising of board of directors and group of executive committee stating their role and responsibilities in each operation department.
When in doubt always ask you manager or colleagues that is why we require this course to run alongside with internship to experience the real-life application.
Q2 - There is evidence of some work being done, but it really required a better effort.
You seem to have misunderstood the question. Your first 3 paragraphs describe a situation with correct Australian Legislation however this answer should be posted in Q3 then you would have gotten a great score for Q3.
You incorrectly defined an Australian Legislation, instead of an Australian Standard! The question asked you to Describe a situation in your internship, or previous work, where you have taken account of an Australian standard in fulfilling your ICT task, and why. If you have not already experienced such situation, describe a situation in your internship where you will need to take account of an Australian standard in fulfilling your ICT task, and why.
Then the 4th paragraph till the end of your answer relates to Australian Standard AS8015:2005 and you list the 6 principles as wrote it as if it is your own words for your future ICT task work. However, these 6 principles used are plagiarized as you did not use Harvard Referencing in your answer for this section. If you are to describe what you will do in the future, ensure to provide example to support each principle to demonstrate you know the real-life application. Your PLAGIARISM Score is very high at 92% hence you get ZERO for this marks. Next time I catch you do this again you will be reported to ACS Education for warning.

I have given tips on this question where it states: “There is no such thing as "The Australian Standard", there are hundreds of Australian Standards, and they all begin with the letters "AS", you must choose an Australian Standard for this answer which begins "AS". This tip has been provided to student so they understand the question better and provide the correct answer.
Now, you see the value of reading the question carefully to get good marks. You scored 1 mark on this question for good attempt.
Q3 - Your work was well thought out. You described a situation. But you failed to identify an Australian Law (legislation) instead you talked about Australian Standard (AS) in general statement.
An Australian legislation usually has the word 'Act' in the title, e.g. 'Workplace Gender Equality Act 2012'. You are allowed to ask your manager or colleague if you do not know which legislation do they follow for compliance.

It probably seems unfair to discount the good work you did. But to put this into context, imagine going to a surgeon. You instruct the surgeon to remove a mole from your finger. She does a beautiful job... of removing your finger! You tell her you are very angry! But she says, "I know I didn't follow your instruction exactly, but I did a very good job!"
Would you agree that her work had merit?
