The report indicates a structured approach to the case study using a technical report format with appropriate appendices. Always start by considering the full purpose of the assessment - “As Group Safety Adviser you are concerned with acting and giving advice on the following aspects of the accident and planning for the new warehouse” *See detail below table as per case study
Consider technical report format. See handout on report writing. In this case it is both a report to management but also must contact technical evaluation so both the ability to influence management and provide the technical basis for your evaluation must be included.
In addition to the main report headings, you must consider how you will structure your sub-headings. I would base this on the methodology you have described in the method section and decided to apply for the accident evaluation (an accident causation model perhaps?!) and then for work planning (a hazard idenfication and risk assessment process perhaps?)

Clear focus on workplace issues, work equipment issues, high risk situations and people with rationale and rich description. A rationale can only be provided if you have used a theoretical basis from which to focus your assessment and evaluation – you will do this by providing the methodological and theoretical basis of your assessment in the ‘method’ section of your report.
Rich description requires that you narratively detail the various issues present and their importance.
In simple terms, this marking criteria is about showing your personal understanding of how to do accident investigation, risk assessment, risk decision making, and risk control processes relative to these various hazard areas.
Evidence that hazards and risks are critically evaluated with reference to current legal, technical and evidence based knowledge. This links to the previous marking criteria in that ‘rich description’ can only be achieved if you are justifying that hazards pose risks based on the legal basis of how they should be managed, broader evidence of how these hazards pose risks to wider industry (for instance), and any other technical information that supports your position from codes of practice or other sources.

Conclusion and suitable recommendations made relevant to the risks and costs of control with an action plan (as appendix). Your conclusion should summarise and synthesis your report against the purpose of your report. It should influence managers to address the issues and hit the main points that must be addressed.
The action plan must be SMART, assign responsibility and seek to determine broad costs as this is what managers are interested in.

Purpose of Case Study Exercise
As Group Safety Adviser you are concerned with acting and giving advice on the following aspects of the accident and planning for the new warehouse:
• Identifying the immediate and underlying/root causes of the accident
• Prevention of further injury to employees;
• Meeting the requirements of RIDDOR, COMAH and CDM notifications or reporting (if required)
• Compliance with the Health & Safety at Work etc Act 1974 and Regulations etc, including any consideration for future potential compliance with Control of Major Accident Hazards Regulations & Construction and Design Regulations
• Reducing potential for compensation claims and liaising with insurers.
• Developing safe construction and demolition policy for the work proposed
• Advising on the legal implications of hazardous agent volume increases
• Preparing an Action Plan designed to minimise the likelihood of a similar accident occurring again and for the safety considerations of the proposed warehouse redevelopment
