As many studies showed that there are many factors influencing health inequalities such as the access to health services, the level of knowledge among healthcare providers and the quality of care between rural and urban areas in Saudi Arabia (ref). In order to tackle these issues, according to ….. Health inequalities will be reduced by improving health literacy. Therefore, identifying the influencing factors is significant in planning an effective health promotion program (chamiah,at al.2015 emen ). unified program that aim to reach all the areas across the country and he need of diabetes educator to lead and support improvement efforts of diabetes self-management is significantly important ( ref) .in the same way, understanding of the impact of low health literacy on chronic diseases prevention and management such as diabetes is crucial. Because health literacy plays a critical role in disease self-management (Ref). Identifying the people health literacy issues and rising awareness how we can overcome the problem for both healthcare provider and people seeking for health service should be consider is priority in police making-decision in Saudi Arabia. According to…. that the higher the educational level of nurses, the more are patient safety and quality care. However, many nurses has lack of knowledge and leadership skills an acknowledging the health literacy issues in diabetic patients (ref). Empowering diabetic patient begins with information and education (ref). Thus, diabetes educator training course will provide a diabetes educator nurse with advanced skills to improving the quality of patient education and improve the health literacy (ref).
Therefore, the prospered service improvement plan is diabetic educator training course qualifying registered nurses to promoting health literacy in diabetic patients. This course is six weeks training course will aim to improve nurse leadership skills and develop an advanced skills and knowledge in low health literacy people with diabetes. Also will enhance the skills and confidence in implementing with the latest approaches in the-self- management education of diabetes. This course will focus in some important points such as communication skills of health professionals and recognize the availability of the human resources of their setting and establish a working network with them. Moreover, empower and motivate the person with diabetes to change his/her behaviour and attitude and successfully manage their diabetes.in additional, Assessing and addressing health literacy in patient with diabetes is the core of this course which allow to nurse’s diabetes educator to tailor the program to the culture and need of the diabetic people with poor health literacy. Finally, diabetic educator could offer advice and promote awareness among the community and offer specialised advice to persons with limited health literacy with diabetes.

The course will be running by professional health care provider whom expert on health literacy issues and diabetic self-management under continues education department and general directorate of nursing in MOH Saudi Arabia. Also the funding will be by MOH in Saudi Arabia represented by continues education department and human resource department.

Monitoring and evaluation is even more imperative in resource poor settings where cost-benefit analysis of interventions is needed most.
The impact of diabetes education is to ultimately improve long-term outcomes in the patients and improve proficiency in diabetes care among the dedicated staff. The evaluation process should be acceptable and friendly, devised to improve people and programmes rather than antagonise them.
The indicators for monitoring and evaluating diabetes education need to be relevant programmes, easy to collect, interpret, and for tracking changes, and preferably universal for comparability within units, regions or countries.
