Assessment item 2: Annotated mind map & bibliography Due date: Part A - Week 9 Part B –Week 10 ASSESSMENT Weighting: 40% 2 To complete Assessment 2 you will need to choose an actual accident or near miss/near hit. Part A Requirements & outputs for submission You are required to: 1. Fully describe the event. 2. Identify significant components of the system/s relevant to the event. 3. Analyse and research the potential human factors issues related to the event (you may find it useful to use the WorkSafeBC model to initially sort the interaction of human factors). 4. Research and then select human factors tools and methods that could be used to investigate the event. 5. Research the strengths and weaknesses of these tools and methods for this application. Outputs for submission: 1. Preamble document which addresses requirement number 1 above 2. Annotated bibliography which evidences the depth and breadth of your review of literature to support requirements numbers 2, 3, 4 and 5 above (example will be provided in moodle) 3. Annotated mind map which evidences your analysis of this problem and includes a. What happened (describe circumstances) – detail can be part of your preamble b. Why it happened (discuss findings) – detail can be part of your preamble c. Cognisance of the nature of the system, system parts, design failures and system life cycle d. Issues and findings related to people, workplaces & management e. Human factors investigation tools recommended to investigate the issues (including identifying strengths and weaknesses of the tool) (the ‘whats’ should draw on evidence in the report and the ‘whys’ should draw on evidence in the report AND in the literature i.e. bibliographies, it should include formal ‘in text’ referencing in the mind map which matches your reference list) 4. Comprehensive reference list Submission: 1. Submit copy of documentation in the ‘sharing drafts’ forum 2. Submit copy of documentation in the assessment submission area for Annotated Mindmap & Bib (please leave as draft i.e. don’t select ‘send for marking’, if you do ‘accidently’ send for marking then just let me know to ‘revert to draft’ so that you can add extra uploads to the area.) Part B Requirements & outputs for submission You are required to: 1. Critique the work of three colleagues against defined criteria. 2. Respond to the critiques you receive. 3. Produce a second iteration of your mind map. 4. Self assess your final mind map. Outputs for submission: 1. Critique (against defined criteria) to three of your colleagues. 2. 2nd iteration of your mind map which includes any new thinking from critique. 3. Covering document that responds to each of the issues raised in the critique and outlines any changes made on your mind map or reasons for not making suggested updates (1-2 pages max). 4. Self-assessment of your final mind map using the criteria. Submission: 1. Your critique to colleagues should be submitted as a reply post in the forum (to author). 2. Your 2nd iteration of mind map and covering document should be submitted in Annotated mind map & bib assignment submission area. We will be discussing these assessment items during the term but if you have any doubt as to what is required for this assessment please don’t hesitate seek clarification in moodle.
