Assessment Title Assignment
Task Description Using the patient safety or quality of care indicator you examined in assignment 2, develop a comprehensive plan for improving care related to that indicator within a nominated health care setting.
Your plan should include an analysis of the issues that contribute to the patient safety or quality of care indicator in the nominated setting. This should also include evidence of the patient experience and any literature available on the experiences of patients related to your chosen indicator.
Please ensure that your clinical reasoning for making decisions about the indicator and the causes associated with it is evident within your analysis.
These decisions should be supported by research evidence and must be clearly described. A Fishbone diagram (and / or use of the London Protocol) must be used to support your decision making.
Once you have established the causes associated with the indicator, develop a plan for improving care related to the indicator in your nominated setting. Use a series of Plan-Do-Study-Act cycles (using 3 cycles) to describe the changes you would initiate. Outline what you would do in each of the Plan-Do-Study-Act cycles to test the changes implemented in each cycle.
Explore your role and the role of health care leaders in improving care related to your chosen indicator.
