Introduction
Cresswell and Sheikh, (2013) presumed that system analysis is an act or process in order to define the purpose or goals of the operations as well as procedures that is accomplishing the business most efficiently. In recent days, there are several advanced technologies are used by the health care organizations significantly. Therefore, management of there system is an important aspects for health care industry (Weaver et al. 2016). According to the provided case study of My Health Record, the mentioned application will be integrated using within the current clinical system for accessing information from one database. This system will design for maintaining patients information along with the monitoring process.
This particular study firmly focuses on explaining the stakeholder roles through designing stakeholder mapping with 4 quadrants. Apart from that, this study also explains about the system through use case diagram and descriptions.
1. Stakeholder Map

3. Questionnaire
3.1 Identification of Stakeholder to send the questionnaire
⦁ Director of Public Health
⦁ Director of Service
⦁ Patients
⦁ Service User
3.2 Introduction Statement for explaining the purpose of the questionnaire
This questionnaire is going to sent among the stakeholders to identify the needs of the automatic system and describes the functionalities of the upcoming system along with its benefits.
3.3 Open and Close ended questions
⦁ Do you want to store data of your patients and analyze it for future strategy?
⦁ Is the information system will be helpful to resolve the current problem at organization?
⦁ Is the information system important to provide more better service to the customers?
⦁ How would you like to access the data that stored into the system?
⦁ Do you like to access the system from everywhere and anytime?
⦁ Do you want to communicate with your important employees anytime?
⦁ Do you show the techniques of patient care to your customers?
⦁ Do you want to restrict the access of data?
⦁ Do you want to create a backup system to avoid future data lost?
⦁ Do you want to view the dashboard of the entire operation when the system start?
4. Use Case diagram and descriptions
4.1 Use Case Model for the Information System

The system checks that the id is authorised
The system provide a list of similar data
The user select the intended data from the list
The system collects the data from the clinical system
The user access the data The system stores the data temporarily in the cloud database
Exception The system is having issues in connecting with the database
Conclusion
This study conducts the analysis report of a system that are going to be implemented at My Health record. The proposed system is an application where the administrative department can track or view the entire operation from any place and anywhere. The staffs are able to login into their system and check the schedule accordingly. Receptionist can track all the patient’s recode in one screen that will help them in providing prompt response to customers query. Moreover, the proposed application is an automated system where patient can register their name and schedule their check up with the specialized doctor.
