General Access to Plant Who has access? How many points of entry? How many easily accessible exits? Are the exit doors large enough to accommodate emergency evacuation? Should all guests need to undertake a safety course prior to entrance? If visitors are allowed, are they always accompanied and no-access areas blocked off and communicated as such effectively? What should the magnitude of the site protection be to prevent trespassers? Is it isolated or close to population? Standard high fencing? If close to large population or bad neighborhood, would electric fencing with many warning signs be a suitable alternative? Is it close to a wildlife habitat of some kind? Are there security system such as guards, cameras? Is the security control centre isolated from busy sections of the plant yet still reasonably accessible? Are the security systems all controlled from one area or control is done through multiple areas? Are there security shutdown buttons? How high clearance should be needed to access button? Would it be safer to have security system for each sector or an overall security system in control of all sections? Fire Safety and Management Are there enough fire exits? Are the locations of the exits well communicated? Would it be necessary to have signals and lights to create a path to exit? Are the doors large enough to allow workers through? Are all sections able to have individual fire exits? Is there fire suppression system in place? In areas that cannot have fire suppression system, is the equipment and infrastructure protected in any way? Is there a sufficient alarm system? Are alarms distributed evenly across the plant? Is there any dead zone where alarm cannot be heard? Would it be necessary to install a different system there or louder alarm? Is drills being performed? What is the overall attitude to the drills? What is the safety culture like at the plant? Do the drills mimic a real scenario as close as possible? How easy is it to access and trigger the alarm? Has the plant pinpointed locations at the highest risk of fire? Is there alarm triggers placed in such areas? Disaster Management: What is the plant’s biggest risk, how severe is the impact and how likely is it to occur? Has preventive measures being enacted? Are they optimal? Are they maintained as well? How often does plant reassess their risk management plan in the case of disaster? What is the proximity to local emergency services? Should there be a planned route for emergency services to arrive quicker, have quicker response time. Is there any on-site emergency services such as firefighting ships for off-shore oil rigs. What are the preventative measures installed? How often are they inspected and how often are they maintained? If they are controlled, where are they controlled; on site or off site? Would there be any possibility for communication breakdown, loss of control, prevention failure? Is there a second line of defense? In the case of a natural disaster, is there on-site backup emergency such as power generators if the plant is reliant upon outside support systems. Is the plant reliant upon outside life support systems to deal with plant disaster such as chemical leak. If there is a simultaneous natural and plant disaster, with no immediate help from outside, can plant disaster be contained? Is there equipment sufficient for the task? Is there the means to coordinate with the Government’s Natural Disaster Management Team? Are the staff/workers trained in disaster management? Is there medically trained personal on site at all times? Is the plant able to withstand earthquake tremor without suffering permanent damage? Waste Disposal Are personal adequately protected? Are there safety features implemented? Are staff aware of the protection and how it works? Is there safety programs offered to train the personnel. Is the handling of waste done properly? Is there proper disposal sites and the associated equipment? Are these disposal sites away from the general public? Does it pose risks to the public? Is the transport of the waste safe and reliable? Is it environmentally friendly as possible? Is the waste disposal site protected by fence, building or facility to not cause potential leakage or spill to environment, population or workplace?
