Information relating to assessment tasks

Your completed assessment tasks will provide evidence for this unit of competency. There are four separate assessments for this unit Part A, Part B, Part C and Part D.

Each task has clear, explicit instructions on when and how to perform the criteria indicated, and what evidence must be submitted by when to be successful.

Each task should be clearly organised and compiled for submission. If it is not and your trainer is unable to understand your submission, you will be asked to resubmit your assessment.

You should keep a copy in case of lost or misplaced submissions.

After reading Chapter 23 Documentation and Recording in your text “Fundamentals of Case Management Practice” please complete Exercise 1: Part II.

Instructions: Read the material on page 409-411, and then write a paragraph of no more than six sentences that covers the following:

  1. The focus of the interview
  2. Your assessment based on a summary of behaviour, appearance, and affect
  3. Any resolution that takes place
  4. The reason for the next contact or the follow-up that will occur

Declaration of Authenticity

I understand this information will be used to assess my competence against the competency standards that relate to the training course. I understand that it will be the basis for the decision to award me a nationally recognised qualification. I am aware that I have the right to appeal either the process or the outcome of this assessment, as set out in the assessment booklet.

All the evidence contained in the assessment tasks is my own work, or partly my own work. Where the evidence is only partly my work I have clearly indicated which part is my work and which is the work of someone else. I have written only what is true.

The following assessment tasks are the work of myself, the student, detailed on this page.
